About the position Nightingale Homecare is seeking dedicated Registered Nurse (RN) Case Managers for both full-time and part-time positions within our home health agency. This role offers a unique and rewarding opportunity to develop one-on-one relationships with patients, guiding them in managing their health issues and celebrating their healing successes. As an RN Case Manager, you will have the flexibility to set your own schedule, primarily conducting patient home visits during weekdays between 8:00 am and 5:00 pm. This position requires a minimum of one year of acute-care nursing experience and a valid RN license in good standing. We offer a competitive salary, with full-time positions earning up to $90,000 annually, along with a generous benefits package that includes a $4,000 sign-on bonus, health insurance, dental insurance, vision insurance, and a 401(k) plan. Responsibilities Develop and maintain one-on-one relationships with patients. , Teach patients how to manage their health issues effectively. , Celebrate patient successes in healing and recovery. , Conduct patient home visits primarily during weekdays. , Complete OASIS documentation with significant support. , Work independently while managing patient care. Requirements Minimum 1 year of acute-care nursing experience. , Registered nurse license in good standing. , Current CPR certification. , Current TB test and Fingerprint Card. , Current AZ driver's license and auto insurance. , Reliable vehicle for driving to client locations. , Ability to work independently.
